INSURANCE & PAYMENT
INFORMATION New patients should provide current insurance
card at initial office visit. Please inform the office staff if there are any changes in insurance coverage, home or work
phone numbers and/or address. We participate with most major insurance plans. There is a number on the back of your insurance card that you may call to see if our physicians participate with your plan. It is the patient’s responsibility to have a referral in place before the first visit. Please check with your primary care physician or your insurance plan to make sure the required referral has been obtained (if necessary).
